Presentation of 2007 data of the National Network for the Vigilance and Prevention of Occupational Diseases (RNV3P)

This article presents data for the year 2007 from the French National Network for the Vigilance and Prevention of Occupational Diseases (French acronym RNV3P), which analyses the main conditions for which experts from Occupational Pathology Consultation Centres (CCPPs) located within teaching hospitals were consulted and for which they diagnosed at least one possible occupational cause. For these health problems at work (HPW), 55% of patients were men and 45% women, with women on average younger than men. Mental disorders and behaviour concerned more than one HPW in four (27%). A relationship between, on one hand the main demographic variables (gender and age) or sector of employment and, on the other hand, occupational diseases, is demonstrated. Tumours were the most common diseases in patients aged more than 60 years, while skin diseases were the most frequent for those less than 30 years of age. According to CCPP physicians, over one third of patients diagnosed with HPW in 2007 were eligible for occupational disease compensation, mainly for diseases of the musculoskeletal system, and mental and behavioural disorders.

This article discusses occupational safety and health among renovators of works of art, ancient artefacts or historical monuments, who are exposed to various hazards: chemicals used in the renovation process; cramped or unsuited work areas, coupled with often awkward postures that involve onsite work; noise; lifting of heavy loads; work at height and risk of falls; stress resulting from urgent orders. The article emphasizes the importance of raising consciousness of the risks involved and informing workers.

CIS 11-0164 Healthy workplaces: A model for action
In October 2009, and after a systematic review of literature on healthy workplaces, WHO held a workshop attended by the ILO, 56 experts from 22 countries, international worker and employer representatives and NGOs in official relations with WHO. The result was the healthy workplaces model that combines evidence-based approaches and principles of health protection and health promotion. The objective of this publication is to promote the model among employers, policy-makers and practitioners. A global perspective of migration and occupational health
Global migration has dramatically increased over the past decade and is at an all-time high, approaching 200 million persons per year. Demographics and economic interdependence suggest that immigration will continue for the near future at record high levels. This article reviews the few studies that have investigated occupational injury and illness rates among immigrant populations. Existing data indicate that higher rates of fatal and non-fatal injuries are common compared to native populations. This increase is in part due to immigrants working in higher risk occupations (e.g., agriculture, construction), but occupational morbidity and mortality is higher among immigrants than native-born workers within occupational categories. Research is needed to identify the causes of increased risk among immigrants and to provide direction to effective public health interventions.

Efficacy of cognitive behavioral therapy training using brief e-mail sessions in the workplace: A controlled clinical trial
This case-control study was conducted to evaluate the effects of cognitive behavioral therapy (CBT) training in improving depression and self-esteem in workers. A total of 261 white-collar workers were assigned to either an intervention group (137) or to a control group (124). The intervention group was offered participation in a group session with CBT specialists and three e-mail sessions with occupational health care staff. Between-group differences in the change in Center for Epidemiologic Studies Depression Scale (CES-D) and Self-Esteem Scale from baseline to three months after the end of training were assessed by analysis of covariance. All subjects in the intervention group completed the group session and 114 (83%) completed the three e-mail sessions. CES-D score decreased by 2.21 points in the intervention group but increased by 0.12 points in the control group. However, between-group difference in change of self-esteem scores was not significant. Implications of these findings are discussed.

Uniform comparison of several drugs which provide protection from noise induced hearing loss
The ability of drugs to reduce noise induced hearing loss (NIHL) has been evaluated in diverse experimental conditions (animal species, noise intensities, durations, assessment techniques, etc.), making it difficult to assess their relative efficacy. The present study was designed to provide more uniform comparisons and to allow a better understanding of the mechanism of the NIHL. The drugs studied included furosemide and the antioxidants N-acetyl-L-cysteine, vitamins A, C, E with the vasodilator magnesium. Mice were exposed to a continuous broadband noise (113 dB SPL for 3.5 hours) and the NIHL was assessed in all animals before noise exposure and 1 week after with auditory nerve brainstem evoked responses (ABR) to broadband clicks and to 8 kHz tone bursts. Each of the drugs alone and in combination led to similar reductions in NIHL.

Organizational interventions for balancing work and home demands: An overview
The effective management of work and non-work demands can have a significant effect on workers' health and performance. The last decade especially has seen an explosion of work-life balance research, resulting in substantial theoretical and practical advances. However, the published literature on organizational interventions to improve work-life balance is extremely sparse. This article provides some prominent examples of empirical research concerning work-life balance organizational interventions and note that these fall into three main groups: initiatives addressing working time and/or working hours, collaborative action research focused on improving workplace equity and performance levels, and initiatives to embed work-life balance within organizational cultures. It also comments on some common difficulties faced by researchers in evaluating interventions. The specific examples presented illustrate challenges to research on the effectiveness of work-life balance interventions. Researchers are encouraged to reconsider the use of merely descriptive rather than more rigorous research designs and to investigate the long-term effectiveness of organizational strategies to enhance work-life balance.

The future of organizational interventions: Addressing the challenges of today's organizations
This article discusses issues to be considered to improve the effectiveness of organizational interventions. First, there is a need to understand how and why interventions work. This calls for an examination of the processes connecting interventions to the desired outcomes. Second, attention should be paid to the appropriateness of interventions. Problems may be difficult to address, for example when they constitute inherent conditions of the job. Third, the use of a quasi-experimental study design does not guarantee a valid picture of the effectiveness of an intervention. For example, control groups may not be comparable to the experimental group, or participants may not be reached by the intervention. Based on these considerations, it is concluded that mixed methods designs are needed.

Exposure to beryllium dust, smoke or vapour may cause berylliosis, a chronic respiratory disease which may evolve towards cardiorespiratory insufficiency. This article explains the radiological and immunological examinations required for diagnosing berylliosis. Furthermore, beryllium is a known lung carcinogen, a fact established by a large body of both experimental and epidemiological studies. The 8h time-weighted average exposure limit in France has been fixed at 2μg/m3. Although berylliosis is recognized as an occupational disease in France, it is often not diagnosed as such and probably remains under-declared. Update of CIS 03-531.

These aspects are summarized in this fact sheet, also available in several other European languages.

Organizational practices and workplace health and safety: A cross-sectional study in manufacturing companies
The results of a mail questionnaire addressed to 312 manufacturing firms in the province of Ontario (Canada) are presented. A cross-sectional analysis of workplace level health and safety policies, practices and attitudes was conducted, based upon lost-time frequency rate (LTFR) data. Lower LTFRs were associated with paternalistic initiatives, recording of occupational health and safety (OHS) measures, greater involvement of workers in decision-making and better managerial attitudes concerning the importance of OHS. Other findings are discussed.

The "Good Work, Good Health" project was aimed at defining good practices on how to improve mental wellbeing of workers. A working group examined the scientific literature on mental health at work as well as policies and practices of a panel of representative European organizations in the telecommunication industry. These good practice guidelines are based on the results of the project. CIS 10-0212 Improving workplace health management in Singapore
In 2005, Singapore adopted a new framework for workplace safety and health. This policy paper is an update of the original framework, taking into account recent developments in the national focus on managing safety at the workplace and the need to look into new and health-related workplace hazards. Two approaches to occupational health management are proposed, namely a compliance-based regulatory framework and a greater focus on eliminating or reducing occupational health hazards. Four strategies are proposed to address different classes (established, emerging and unknown) hazards: targeted intervention programmes; monitoring and intelligence gathering; research and standards development; promotion of good practices.

Making the link between health and productivity at the workplace - A global perspective
This article discusses the relationship between health and productivity at the workplace by reviewing the current status of the fields of workplace health promotion and health management. The prevailing chronic disease trends coupled with economic pressures have proven to be significant challenges for employers and employees alike. While an overall progress in workplace health promotion can be observed, the number of companies which take a proactive and integrated approach to workplace health remains small. Workplace health promotion programmes in the United States typically focus on the individual health risks of employees while their European counterparts target physical and, more recently, psychosocial hazards. A number of specific tools and programmes for integrated health management are described, including self-report instruments to measure presenteeism.

Laboratory study of selected personal inhalable aerosol samplers

Assessment of inhalable dust exposure requires reliable sampling methods in order to measure airborne inhalable particle concentrations. Many inhalable aerosol samplers are available but their performances widely vary and remain unknown in some cases. The sampling performance of inhalable samplers is strongly dependent on particle size and ambient air velocity. In this study, five inhalable aerosol samplers were evaluated in two laboratory wind tunnels using a polydisperse glass-bead test aerosol. Findings are discussed. Significant differences between moving air and calm air sampling efficiency were observed for all the studied samplers.

Each year in the United Kingdom, 6000 new cases of lung cancer are diagnosed and 4000 deaths occur from chronic obstructive pulmonary disease, due to occupational exposures. This article outlines the efforts being undertaken to address the thousands of deaths and cases of ill-health caused by airborne hazardous substances through a new scheme aimed at ensuring that respiratory protective equipment is fitted, worn and maintained in good condition.

Economic incentives as a policy tool to promote safety and health at work
This review article discusses various types of incentives for promoting occupational safety and health (OSH) in light of economic theory and evidence from research. Governments can subsidize employers' investments in OSH with subsidies and tax structures. These incentives are successful only if the demand for OSH responds to the change in the price of OSH investments and if the suppliers of OSH are able to increase their production smoothly, failing which the subsidy will only lead to higher prices for OSH goods. Both public and private insurance companies can differentiate insurance premiums according to past claims. There is evidence that this can effectively lower the frequency of claims, but not the severity of cases. Policy implications of these and other findings are discussed.

Developing guidelines for good practice in the economic evaluation of occupational safety and health interventions
One of the objectives of a workshop held in 2009 in Amsterdam, the Netherlands was to discuss methods for the economic evaluation of occupational safety and health interventions at the corporate and societal level. This article reviews the contributions made at the workshop and proposes framework principles and a set of recommendations to serve as the foundations for developing guidelines for good practices in this area.

Transferring results of occupational safety and health cost-effectiveness from one country to another - A case study
There are a limited number of studies on the cost-effectiveness of occupational health and safety (OSH) interventions. Applying the results of a cost-effectiveness study from one country to another is hampered by institutional differences in national health care and social security systems. In order to find out how these problems can be overcome, this study transferred the results of a Dutch occupational cost-effectiveness study to the Finnish situation and vice-versa. Incremental cost-effectiveness ratios (ICER) were calculated for the target country, allocating costs to employers, employees and taxpayers. It is concluded that results of cost-effectiveness studies can be transferred from one country to another, but many adjustments are needed. An extensive description of the intervention, a detailed list of resource use, allocation of costs to various parties and detailed knowledge of the health care systems in the original studies are necessary to enable calculations.

Future engineers, architects, medical professionals and business administrators and managers will all need to take account of occupational safety and health (OSH) in their working lives. This report presents a variety of cases concerning how OSH has been included in university-level education in the European Union. However, relatively few examples were found where OSH education had been truly embedded within the curriculum of other undergraduate studies, such as a general engineering undergraduate course or a business studies course. courses. By highlighting cases from EU Member States, this report aims to provide practical support for the development of OSH programmes in university education.

Development of an assessment tool for repetitive tasks of the upper limbs (ART)
Health and Safety Executive and Local Authority inspectors play an important role in preventing work-related musculoskeletal disorders (WRMSDs). They require assessment tools that offer an intuitive and relatively quick process to screen workplaces for high-risk activities, raise awareness of risk factors, demonstrate the presence of risk and recommend areas for improvement. The Manual handling Assessment Charts tool (MAC) has been recognized as a useful process to screen workplaces for high-risk manual handling operations. The MAC tool is designed to identify and help assess high-risk manual handling operations. However, its scope is limited to manual handling of heavy items, which primarily present a risk of lower back disorders. There was a demand for a similar tool that inspectors can use to screen more frequent handling of light loads or other repetitive tasks and the common physical risk factors in work that may contribute to upper limb disorders (ULDs). This led to the development of the assessment of repetitive tasks (or ART) tool described in this report.

Developing the management standards approach within the context of common health problems in the workplace: A Delphi study
The objective of this research project was to provide evidence, arguments and recommendations in relation to the development of a more unified framework for the HSE's programme on health, work and wellbeing. Essentially, it was to determine whether the management standards approach could be used more widely to address the most common health problems at work. A better understanding of the current strengths and weaknesses of the management standards approach and its potential had to be developed. This need was addressed using a Delphi methodology, framed by a focussed review of the relevant scientific and professional literature, to elicit, harvest and explore expert knowledge in this area. Findings are discussed.

Lessons learned from the Large Organisations Partnership Pilot (LOPP)
The Large Organisations Partnership Pilot (LOPP) was a joint initiative, launched in October 2005, between the Health and Safety Executive (HSE) and the Local Authorities Coordinators of Regulatory Services (LG Regulation) to explore how engagement with large organisations having more than 10,000 employees in the United Kingdom with multi-site operations could be improved. The objectives of LOPP were to present a more coherent and coordinated face to large organisations, to secure improvements in safety and health outcomes in the participating organisations and to give participating organisations an early voice in discussions on emerging policy areas such as reward and recognition and alternative penalties. The experience over the three years provides a useful database for drawing practical lessons and determining the factors which influence the establishment of LOPP-style relationships for any future HSE/LA engagement with large organisations.

The effect of individual job coaching and use of health threat in a job-specific occupational health education program on prevention of work-related musculoskeletal back injury
The objective of this study was to examine the effect of individual job coaching and use of health threat in a job-specific occupational health education programme in preventing work-related musculoskeletal back injuries during manual materials handling in construction labourers. Two hundred five Hong Kong labourers were recruited by 3-stage cluster sampling process and randomly assigned to receive different education programmes on manual materials handling. The control group was given a conventional programme, whereas the experimental group received a job-specific programme. A statistically-significant group difference was found in the knowledge and practical skills on manual materials handling after training. Furthermore, the results from 1-year cumulative incidence revealed a significantly lesser number of first-time reports of work-related musculoskeletal back injuries in the experimental group. These results indicate that job-specific education is an effective preventive strategy for work-related musculoskeletal back injury.

A longitudinal study on the relationship between weight loss, medical expenditures, and absenteeism among overweight employees in the WAY to health study
The objective of this study was to quantify the extent to which successful weight loss among overweight or obese employees translates into subsequent savings in medical expenditures and absenteeism. The analysis relied on medical claims and absenteeism data collected from overweight and obese employees at 17 community colleges in North Carolina, USA. There was no evidence that participants achieving at least a 5% weight loss experienced reduced medical expenditures or lower absenteeism during the 12-month weight loss intervention or in the subsequent two years. These results suggest that a quick return on investment from weight loss programs, even effective ones, is unlikely. Nevertheless, as with other employee benefit decisions, the decision about whether to offer weight loss programs should take into account many factors, such as employee health, in addition to the potential for a quick return on investment.
