The use and cost of first aid in the food and drink manufacturing industry [en inglés]
Los resultados de una encuesta por cuestionario, realizada en 36 plantas de fabricación de productos alimenticios y bebidas en el Reino Unido, señalaron que la casi totalidad de ellas había respetado la reglamentación en vigor, poniendo a disposición de sus empleados los equipos de socorro formados a tal efecto, los equipos y locales apropiados. El coste medio anual se elevaba a 16.000 libras esterlinas por trabajador. La mayoría de los tratamientos administrados (un 93 %) fueron de carácter menor. Se recomendó la reducción del contenido de los cursos de socorrismo pero aumentando su nivel de frecuencia para asegurar la intervención eficaz de los socorristas en una emergencia médica.

Este caso pone en evidencia numerosos aspectos que deben estudiarse en similares circunstancias, incluyendo la eficacia de los trajes de protección, la descontaminación del paciente antes del transporte aéreo y la opción de un transporte por carretera como solucion alternativa.

The health status of an ambulance service
The health status of 93 male ambulance personnel in Belfast, United Kingdom was evaluated; risk factors for coronary heart disease were assessed. Blood pressure values were significantly higher than in a comparable study of the local general population; the incidence of self-reported smoking was also higher. The majority of body mass indices were above the acceptable range and serum cholesterol levels were raised in 52% of personnel. Overall, 83% of men over 40 and 49% of younger participants had two or more major coronary risk factors. Results suggest an urgent need for strategies to reduce overall risk.

The results are presented in tabular form. Preliminary conclusions: if water is administered immediately after a chemical splash accident, there are no harmful consequences; the essential aspect of first aid is immediate administration; the product Diphotérine R is equally effective against acids and bases; it is important to understand how Diphotérine R works: mixing it with water seems to have no beneficial effect. This report describes a survey of the retention of knowledge of first-aiders. Results suggest that first-aiders retain only a percentage of what is learned on first-aid courses and that few first-aiders update and refresh their memories by looking at the first-aid manual between refresher courses. More work needs to be done to identify the cause of low retention of learned material.

Impact of the new European legislation regarding vocational driving on London Transport Medical Service: The doctor's role
The outcome of the first 1000 medical examinations undertaken by London Transport Medical Service (LTMS) to comply with the regulations for non-passenger carrying public carriage vehicle licence holders (primarily bus and car mechanics and electricians) was analysed to assess where the primary role of the examining doctor affected the final outcome. It was found that the primary involvement of the doctor possibly altered the outcome in only two cases. In 49 cases, the medical practitioner had an interpretative role in the final assessment, or in the pursuit of further information, such as clarification of past history from other sources. In these cases, actual contact with the applicant did not affect the final outcome.

Revised version of the 1989 issue of this publication abstracted under CIS 90-784. This illustrated booklet gives practical advice on the steps to be followed in providing first aid in case of traffic accidents, avalanches, electric shock, falling through the ice of a frozen river or lake, and other accidents. Examples illustrate the different phases of the procedures: removal from danger, taking off helmets of injured persons, unconsciousness, respiratory arrest, bleeding, injuries, burns, fractures, poisoning, etc. This booklet may be used for training purposes.

Offshore safety and effective psychological functioning - A case for safety cells
The effects of environmental conditions on an individual's ability to make effective decisions about safety and escape during a major disaster are discussed with reference to the 1988 Piper Alpha offshore platform disaster. The destruction of the normal command and control function is also discussed. The concept of the Safety Cell, a work group assuming command and control for its own safety and escape within the overall safety system of the platform, is outlined along with the need for appropriate survival training within each group. Such a system would provide a form of psychological protection to the individual in a situation of extreme stress.

Work stress in emergency medical technicians
Emergency medical technicians (EMTs) participated in a union-sponsored study to better understand the dynamics underlying their high turnover rate. Three questionnaires were used: the Occupational Stress Index, which assesses stress, strain, and coping; the Staff Burnout Scale for Health Professionals; and a job satisfaction survey. The sample had high stress, strain, and burnout scores. Coping skills were within the normal range. Burnout, stress, strain, and coping were significantly related to job satisfaction, worry about infectious diseases, and perceptions of poor treatment by emergency room personnel and fire fighters. The areas of greatest job dissatisfaction were: low pay; lack of comfortable quarters; inadequacy of equipment; administrators' unfamiliarity with the work-related demand and stressors faced by EMTs; the risk of developing health problems.

National medical response to mass disasters in the United States
Preparing for a resuscitation response to mass disasters requires revision of present local, regional and national disaster plans in the USA. These should include: life-supporting first aid and basic rescue capability of the lay public; advanced trauma life support and rescue capability brought quickly to the scene from local and regional emergency medical service systems; trauma hospitals sending medical resuscitation teams to, and receiving casualties from, the disaster scene. It is proposed that the National Disaster Medical System replace its civil defence model with an emergency medical services model designed to mobilise rapid support for local emergency services systems from regional, state and national resources.

A human factors approach to the effective design of evacuation systems
Various design factors of evacuation systems with particular reference to fires on board aircraft and offshore installations are discussed. Human factors such as the potential for panic, confusion, ignorance and disorientation and the implications for evacuation system design are considered. Proposals made by various authors to produce models of human behaviour under extreme threat stress factors which may affect muster times following an alarm signal. The main consequences of the Piper Alpha offshore platform incident are summarised. Proposed hardware solutions and human behaviour and ergonomic issues of the evacuation procedure are discussed.
