Occupational injuries in the agricultural sector - A statistical approach - II. Occupational diseases

Sequel to a first article abstracted as CIS 79-284. A statistical analysis (covering 3,276 cases of occupational diseases compensated during the period 1974-1976) of the frequency of occupational diseases with breakdown, following the European classification, into 5 groups of cases: infectious and parasitic diseases (82%), skin disorders (8%), poisoning due to chemicals (6%), respiratory diseases (2%), postural complaints (0.6%). Brucellosis alone accounted for 74% of the total number of cases studied. The article concludes with a review of French regulations concerning compensation for, and the prevention of, occupational diseases in the agricultural sector.

Pathogenesis and prevention of pneumoconiosis due to "sendo" - 1. Working conditions of mat production; 2. Epidemiological study; 3. Experimental studies on fibrogenicity of "sendo" dust.
Rush matting is an important farm product in Japan, the raw material of the traditional "tatami" mat. At rush harvest workers are exposed to high concentrations of "sendo" (clay containing quartz, chlorite and minerals, with a free silica content of about 20-30%), which is used to dye rush, and they are also exposed to lower concentrations of "sendo" during weaving of matting. In Part 1, results of occupational hygiene studies are reported. In the health survey of rush farmers (Part 2), 44 cases of mild fibrosis and 37 suspected cases of pneumoconiosis were seen in the chest X-rays of 326 examinees. Length of exposure to "sendo" dust was related to roentgenographic findings. Part 3: Slow but steady progress of collagenous fibrogenic reactions was observed in the rat lung when "sendo" suspension was infused intratracheally. This suggests that pneumoconiosis in rush farmers may be caused by inhalation of "sendo" dust. Studies were performed with 3 substitute materials, consisting mainly on ZnO, CaCO3, BaSO4 and MgO, with a free silica content of <1%. These materials reduced the dust hazard in experimental studies.

Diagnosis of allergic alveolitis of the "farmers' lung" type in the light of case studies
Rozpoznawanie alergicznego zapalenia pęcherzyków płucnych typu "płuca rolnika" w świetle własnych obserwacji [in Polish]
The authors examined a group of 370 farmers with chronic lung disease and diagnosed 20 cases of "farmers' lung", using the following criteria: evidence of antibodes to Micropolyspora faeni found in serological tests, anamnesis disclosing exposure to plant dust and connection between the exposure and the disease, presence of characteristic respiratory and general symptoms. Combined appearance of precipitins and of antibodies (complement fixation reaction with M. faeni is of great diagnostic significance). This combination of antibodies was found in 19 out of 20 patients with farmer's lung (95%), in 48 out of 142 subjects with granulomatous lung inflammation (34%), and in 34 out of 208 remaining patienets with other chronic lung disease (16%). The differences between these groups were statistically highly significant. It should be noted, however, that at least in some positively reacting patients with granulomatous lung inflammation, the asymptomatic form of farmer's lung can sometimes occur, associated with other chronic lung disease.

The results of occupational hygiene and ergonomic analyses of self-propelled agricultural machinery and tractors and epidemiological studies in agricultural machine operators are presented as an occupational hygiene work profile. Conclusions are drawn for prevention of occupational injuries (especially hearing, respiratory and digestive disorders), for regular occupational medical supervision, and improvement of health care in agricultural workers.

Raynaud's syndrome in chain-saw operators - Deductions from statistical studies

Statistics on 4,074 agricultural workers show more than 5% of them to suffer from Raynaud's syndrome. In chain-saw operators the prevalence is 45%. Detailed analysis of the cases brought out the role played by duration of exposure and saw weight, confirming the traumatic aetiology of many cases of Raynaud's syndrome, supported objectively by the topography of the damaged vessels. Hypotheses are presented concerning the causation of vibration-induced Raynaud's disease. Preventive measures are: limitation of duration of exposure and weight of the equipment, use of anti-kickback chains, fitting of an open vibration-damping insular handle with a triple-layer sleeve of rubber, semi-hard foam and soft plastic. The study is summarised and supplemented by a clinical article by the same author appearing on p.405-408 of the same issue.

Exposure to biological dusts in agriculture

The air of cow stables and grinding sheds, and during indoor forage threshing and grinding, was studied in 2 farms in a region where farmer's lung is a serious problem. The highest exposure to organic dust occurred during handling of hay and grains. Cladosporium, Rhodotorula, Actinomyces and Penicillium spores were the most commonly found. Spore concentrations of thermophilic moulds were higher during handling of baled than loosely sampled hay. Spore concentrations were 100-800 times higher during indoor threshing, grinding and handling operations that in the outside air, and the mean spore concentration was 300,000/m3, with peaks 10 times this figure. Dust concentrations were above the Finnish exposure limits (10mg/m3) in all work processes studied. During grain grinding 84% of dust particles were respirable.

Studies on exposure to dinitro-o-cresol (DNOC) in agricultural chemists

These studies were undertaken in 1975 and 1976 on pesticide exposure resulting from the manufacture and application of the spray solution. Aspects considered were exposure of workers to pesticides in general, the relative amount of DNOC exposure over the year, determination of workplace concentrations where mobile application methods are used, and blood level determinations. A case of poisoning is described. Measures to reduce the health hazard associated with exposure are set out.

Overview of occupational exposure to microbial agents in the air of animal housing quarters

The veterinary and occupational medical literature, above all of Eastern European origin, on microbiological agents in the air of poultry, swine and cattle housing quarters is collected and evaluated from the occupational hygiene viewpoint. It has not yet been established in what measure microorganisms represent a hazard for animal husbandry workers. Aspects considered: air pollution of animal housing quarters by microorganisms, factors influencing the microbial content, determination of bacterial counts, findings for various types of animal quarters, types of organism, use of nutrient antibiotics and bacterial resistance, respiratory tract diseases. The main areas of occupational hygiene and medical research in this field are outlined.

Hearing change in drivers of agricultural tractors

MD thesis. When considering occupational disease in agricultural workers, it should be realised that hearing change occurs not only in tractor drivers but in many others whose work is becoming more and more mechanised and therefore more and more noisy: workers in sawmills, dairies and flourmills, timber workers, etc. Following a review of the anatomy and physiology of the organs of hearing, the phenomenon of noise and general considerations on hearing loss, the author gives the results of a comparative audiometric study of 300 tractor drivers divided into 3 age groups: bilateral hearing damage with scotoma at 4,000Hz; maximum hearing loss between 35 and 50 years, especially in the higher frequencies; proportionally less significant hearing loss between 50 and 65 yeas; the hearing loss is greater in the left ear, which is usually more exposed to engine noise. Reference to French, German and Italian standards concerning noise; noise measurements taken on engines and during agricultural work; preventive measures; compensation. A table of noise-induced occupational diseases in agricultural workers is proposed.

Brucellosis (Bang's disease) considered as an occupational disease

This article gives a roundup of the various aspects of this disease, referring to data in the literature and to 700 case studies of occupational brucellosis (mostly caused by Brucella abortus) observed by the author in 30 years of practice. All the possibilities offered by bacteriology, immunology and serology should be used, as the symptoms of brucellosis vary considerably. The results of serological tests are considered doubtful without a clinical examination. The relations between infection, clinical symptoms and diagnostic results are shown in a synoptic table showing 11 different types of course which brucellosis caused by Brucella abortus can take in man; occupational factors in pathogenesis are indicated. The author emphasises the importance of Burnet's intradermal reaction test. The article concludes with considerations on: treatment, statutory compensation, and medical prevention in the author's veterinary laboratory.

Determination of minimum period to be observed before re-entering greenhouses after applying pesticides

Research on the decomposition of pesticides in water used for washing cucumber leaves and horticultural workers' hands. Residual amounts of actellic, unden (arprocarb) and endodan were determined by thin-layer chromatography, and of zineb by colorimetry. Reproduction of the results, which show that actellic, endodan and zineb undergo rapid decomposition in greenhouses, whereas unden is more stable (diminution by only 16.7%, as measured on leaves 4 days after treatment). The authors recommend that a period of 4 days should be allowed to elapse before re-entering greenhouses, and that unden should not be used in greenhouses.

This article analyses the overall French statistical returns for 1974-1975 and 1975-1976, covering some 3 million persons engaged in agricultural activities (750,000 of them wage-earners). A general section is devoted to the data available concerning 300,000 occupational accidents involving wage-earners and self-employed farmers, giving a breakdown of the number of accidents in relation to the size of the workforce, number of hours worked, site of injury and accident agency. Section 2 gives a breakdown of the data by branch of activity (crop farming and livestock rearing; logging operations; mixed farming, market gardening, landscape gardening etc.; rural handicrafts; storage, supply, processing and marketing of agricultural produce; tertiary sectors in agriculture). The results are shown in synoptic tables.

Incidence of antibodies against microorganisms from plant dust in farmers with chronic lung diseases

Serological investigations by use of antigens derived from Micropolyspora faeni and Erwinia herbicola were carried out in 370 hospitalised farmers with chronic pulmonary diseases. Others (609 subjects total) with and without a history of exposure to dust, "healthy" farmers, "healthy" cotton industry workers and blood donors without any history of dust exposure were also tested. Tests used were: gel precipitation (double diffusion) (DD), complement fixation (CF), haemagglutination (H) and passive cutaneous anaphylaxis (PCA). Precipitins to M. faeni were found in 36.2% of farmers with chronic disease, significantly higher than in the other groups. Farmers had also a high proportion of reacting subjects in the CF, H and PCA tests with M. faeni antigen (47.4%, 63.0% and 52.0% respectively). A significant correlation was demonstrated between preciptin and CF reactions. In the farmers, reactions were obtained in 47.3%, 55.4% and 48.8% in the DD, CF and PCA tests, respectively. These reactions were significantly correlated with those to M. faeni. The results point to the importance of antigens in these dusts in the aetiology and pathogenesis of pulmonary diseases in farmers and need for measures to limit exposure. English summary.

A swivelling seat to improve tractor drivers' posture
A mechanism that allowed a tractor seat to swivel by up to 20 degrees from the normal forward facing position was tested. Electromyographic techniques were used to monitor muscle activity during static laboratory experiments and angles of body twist were studied photographically. There was a decrease in muscle activity in the shoulder and neck regions when the seat was used. The potential benefit of swivelling the seat was not fully utilised by the subjects. Subjective tests at 3 farms confirmed that the seat was of benefit to the driver, particularly for tasks requiring him to look backwards frequently.

Results of studies in man to determine the olfactory threshold (0.026mg/m3 in the most sensitive subjects) and the threshold for an effect on cerebral bioelectric activity (0.016mg/m3) of this organophosphate pesticide. A concentration of 0.01mg/m3 is recommended as the limit for brief exposure. Animal experiments also yielded an exposure limit of 0.01mg/m3 for daily exposure in hot conditions.

Serious occupational accidents in agriculture

Accidents causing more than 60% invalidity were analysed by interviewing a sample of 214 victims. The largest group of accidents, 19.6%, took place on floors, stairs or walkways; with tractors 16.4%; falls from a height 15.4%; other falls 14.0%; and handling animals 7.0%. Machine accidents occurred mostly with men, and animals were often involved in accidents with women. More than half the accidents occurred during harvest; the weekday was not significant. Most injuries are to limbs and 53.7% of all are fractures (22.0% cuts). Hospital stays averaged 51 days. Most farmers believed safety information is lacking.

Fictitious safety in agriculture

Report and results of an audiometric survey carried out in the Netherlands, covering 63 tractor and/or combine harvester drivers who underwent, over a period of 3 years, audiometric tests before and after the ploughing and harvesting seasons. A comparison of hearing threshold shifts in farm workers and workers in metalworking shops showed that the hazard of hearing damage was about the same in mechanised farming and in the metalworking industry. The authors recommend that farmers should choose the least noisy types of tractor when renewing their equipment, and should wear earmuffs.

Based on a case study of a fatal accident, this article stresses the hazards of using motor mowers or 2-wheeled tractors on steep slopes. Recommendations: use only machines equipped with an efficient, easily-operated handbrake; never change gear when guiding the machine straight uphill or downhill; before changing gear, turn the machine parallel to the contours.

Working and environmental conditions in the rush (igusa) harvesting process.
Harvesting of rush is hard physical work, and the workers are exposed to the scorching heat of the sun from 6a.m. to 9p.m. Mean values measured in the rush cutting field: dry-bulb temperature 29.7°C; relative humidity 74.3%; wind velocity 1.6m/s; globe thermometer temperature 39.8°C; WBGT index 29.3°C; estimated heat stress index (modification of Belding and Hatch) 91.9 for men and 96.4 for women; predicted sweat rate for 4 hours 4.44l in men and 4.11l in women. No cases of heat stroke were observed. Measures recommended to control heat stress are arrangement of a suitable work-rest schedule and mechanisation of arduous jobs.

Nitrogen dioxide-induced pulmonary disease - Five new cases and a review of the literature.
This study combines the results of examinations of 23 mainly agricultural workers (18 with a transient upper respiratory tract syndrome, 5 with pulmonary oedema or bronchiolitis obliterans) and findings in the literature. The formation in silos of NO2, which is present at levels much higher than permissible industrial exposure limits, is described. Conclusions: NO2 exposure is commoner than is generally appreciated; the fatality rate is high (29% for silo filler's disease); steroids are effective and should be continued for at least 8 weeks; some patients may develop persistent functional changes; there is no evidence that long-term exposure to low NO2 concentrations produces chronic airway obstruction; preventive measures could eliminate NO2-induced pulmonary disease.

The mortality rate (1959-1961) from respiratory diseases in this group was 3 times higher in employed workers than employers or self-employed workers. Accidental deaths were also higher in the employees. Hazards in agriculture that could contribute to accidents and respiratory disease are reviewed.

Other requirements: reliable fastening of the cutter knives, which should be able to resist unexpected strain; reliable clutch coupling of the straw chopper operated from the driver's cab; protective shields for retractable straw choppers; warning notice and maintenance instruction.

CIS 78-1633 Chemical compounds used in agriculture and food storage.
This set of loose-leaf sheets contains recommendations for safe use of about 300 organic and inorganic substances. The sheets are produced periodically. Health status of workers applying pesticides in greenhouses

Results of medical studies in 72 workers (67 women and 5 men) growing flowers and vegetables in hot-houses, with a length of service of more than 5 years, and a control group of 30 persons matched for age and sex. The main findings were autonomic and gastrointestinal disturbances. Examinations 3h after application of an insecticide and a fungicide showed a significant fall in serum cholinesterase and blood peroxidase activity, haemoglobin, total cholesterol and urinary 17-ketosteroids. Poisoning was due mainly to skin absorption. The combined action of the 2 substances is considered responsible.

Asthma in a rural environment - Epidemiological study

An epidemiological study in 10,602 subjects living in an exclusively rural region employed in fodder production, cultivation of cereals and animal husbandry indicated a high percentage (7.38% or 773 subjects) of asthmatics. The most frequent allergens were: house dust, acarids, cereal dust, dander, feathers, pollen, hair, nails etc., of animals. Cereal dust and animal products are the most specifically rural. The rate of multiple sensitivity is 60%. There was only low sensitivity to industrial products for agricultural use (herbicides, pesticides and fertilisers).

Rheographic studies in 30 young women after a day's work on various types of agricultural tractors showed that low-frequency vibration causes circulatory changes (venous hypotension and blood stasis) in the brain, bone marrow and pelvic organs. The same changes occurred in laboratory exposure to low-frequency vibration in 11 healthy women not exposed to vibration in their work. The changes were more marked during the menstrual period.

Onychopathy in mushroom growers

Following the replacement of wooden crates by plastic bags in the mushroom-growing industry, the authors observed 5 cases of changes in the fingernails of mushroom growers: onycholysis with wearing down of the lateral edges of the nails, koilonychia, longitudinal splitting and filiform haematoma. The negative results of mycological examinations (in spite of the presence of contaminants) and of patch tests show that these lesions are not due to mycotic infection or to any chemical agent, but are traumatic, due to the repeated long-term rubbing of the nails when lifting and carrying heavy plastic bags. This nail condition might be found in garbage disposal workers who also have to handle plastic bags.

Physiological work load in farm work with powered cultivators.
The walking tractor is still widely used in mountainous villages in Japan. Subjective fatigue symptoms of farmers after cultivating with this machine in paddy fields were mainly localised in the shoulder, back and limbs. Energy consumption during the work was 4-7cal/min according to the walking speed of the operator, which depended on the type of attachments used and soil conditions. From the relationship between heart rate and oxygen requirement the static muscular load seemed to be considerable during machine operation. Polygraphic studies of physiological functions while driving a trailer with the tiller type cultivator indicated considerable mental strain. Summary in English.

Strawberry pickers' foot drop.
A series of letters to the editor reporting cases of this condition following a prolonged period of strawberry picking. The condition presents as lateral popliteal nerve palsy. Seppäläinen et al. (p.767) report on a number of occupational cases in Finland. They estimate that 0.4% of professional strawberry pickers experienced the palsy in the summer of 1975. Permanent damage may result from the condition. The picking position should be changed as soon as the first symptoms - tingling, etc. - appear.

Results of a survey based on interviews, clinical ear examinations and audiometric tests: degree of mechanisation and noise exposure; measurement of noise from tractors and chain saws; classification of hearing loss. Out of the 186 agricultural workers examined 9.7% only had normal audiograms. A comparison between building and agricultural workers showed that hearing damage was more common among the latter. Only a small percentage of agricultural workers used hearing protectors. (English summary.)
