Permanent disability following occupational accidents according to type of injury, affected region and selected risk indicators in Mexican oil industry workers

Between 1990 and 1993, 8,638 occupational accidents were recorded in the Mexican oil industry and 1,685 workers (19.5%) had some degree of permanent disability. The most frequently injured parts of the body were the hands and wrists, and the main sequela was amputation. The incidence of lesions was higher in men than in women; this can be explained by the fact that women are not assigned to high-risk activities. A linear increase of incidence with age was observed. The study shows a high proportion of injured workers with high degree of disability.

Adaptation of a workplace

This article presents an ergonomic analysis of the workplace of a mentally handicapped worker. His main task is the printing of saw blades on a semi-automatic machine, and incidentally to clean the printing plate and stamp when he sees a defect and to clean the machine at the end of the day. The proposed changes are aimed at improving work posture as well as working conditions (lighting, fan, workbreaks).

Structural anthropometric measurements for wheelchair mobile adults
Structural anthropometric measurements for males and females were determined for wheelchair-mobile adults. A photogrammetry methodology was used to obtain the measurements. The various subject demographics including age, level or type of dysfunction as well as the specified anthropometric dimensions of the wheelchair-mobile subjects were identified. The data are useful for the design of industrial workstations for wheelchair-mobile adults. Present workstation design principles based on seated able-bodied anthropometric measurements would not be suitable for this population.

A study on job placement for handicapped workers using job analysis data
This study determined job titles which can be adequately performed by various types of disabled workers. For each job title, 41 specific variables, falling in three major classes (required education and experience, physical demands, task environment) were factor analysed, using principal components analysis with Varimax rotation. The five-factor (occupational hazard, verbal communication education and training, visual acuity, body agility and manual ability) solution explained 58.4% of the variance. Applying the hierarchical clustering procedure to the loading scores on the five job factors, the 112 job titles were classified into 15 homogeneous clusters. By mapping the loading scores on the five job factors and the handicap profile developed, job titles which can be adequately performed by visually impaired, wheelchair dependent, learning disabled, mental retarded or hearing impaired workers or by individuals with emotional disturbances can be classified. The results can be utilized to expand the perspectives of both the counsellor and the person being counselled about job possibilities and job requirements.

This information note on workplace design for disabled workers describes how to make workplaces comply with article 13 of Royal Decree 486/1997 (see CIS 97-1067). Construction of architectural barriers should be avoided, and space and work equipment should be adapted through inexpensive changes, to the needs of disabled workers. It is important that ergonomic criteria be taken into consideration to adapt the workplace to the working capacity of those employed there. Emergency planning must also take the needs of disabled workers into account.

Variability of geometric and mechanical measurements on wheelchair users
Mesure de la variabilité des paramètres géométriques et mécaniques de la position des personnes en fauteuil roulant [in French]
A measurement method was developed to describe and quantify the handicapped user's 3D position in his wheelchair. Pre-defined landmarks are digitized with an articulated mechanical arm. To quantify the pressure distribution, a system composed of sensors is used on the seat and the back of the wheelchair. In this method, different types of errors contribute to modify the geometric and mechanical measurements. The purpose of this study is to evaluate these errors and their impact on the precision of the various parameters on a sample group of five non-handicapped subjects. The variability of most of the geometric parameters was <2° where the sagittal rotation of the pelvis (3.8°) showed the highest variability and the thigh angle (0.5°) the lowest. The lowest variability of the mechanical parameters was obtained for the average pressure and pressure gradient measurements. It was confirmed that the geometric and mechanical measurement method presented in this paper is a non-invasive, simple and accurate procedure for seating evaluation.

Occupational injuries among older workers with visual, auditory and other impairments
An attempt was made to validate a previously defined model of risk of occupational injuries among older workers with visual, auditory or other impairments. That model, based upon the Health and Retirement Study (HRS), was recalculated using data from the 1994 National Health Interview Survey (NHIS). The parameter estimates for impaired hearing (1.81 in NHIS, 1.55 in HRS), impaired vision (2.42 in NHIS, 1.48 in HRS) and self-employment (0.22 in NHIS, 0.49 in HRS) were in the same direction and of roughly the same magnitude. The previously defined model was confirmed using NHIS data. The data suggest that as the workforce ages, more attention must be paid to the accommodation of disabilities in the workplace, especially sensory impairments. Risk management of electromagnetic compatibility with medical devices
A risk management approach to the problem of electromagnetic interference with medical devices worn by workers is presented. Such devices include hearing aids, pacemakers, infusion pumps and wheelchair controls. Applications of radio-frequency radiation which may interfere with these devices include radio communications, mobile phones, TV and radio broadcasting, radar and navigation systems. Risk analysis and assessment issues are discussed and methods of risk control and communication are outlined.

These Regulations came into force on 5 Dec. 1996. They oblige employers to make special provisions for the protection of disabled (handicapped) workers, e.g. by providing information in braille, sign language, large print etc. Barriers, guardrails and warning signs must be evident to disabled workers. Monitors must be appointed to assist persons in need of special assistance in case of emergencies.

This editorial discusses issues surrounding the provision of compensation for individuals unable to work through disability while excluding those considered to be unworthy of benefits. Problems associated with the determination of disability are examined, with particular reference to regional musculoskeletal symptoms. It is argued that in order to surmount the preoccupation with the unworthy, the concept of disability should be demystified. It should be recognized that individuals with lesser degrees of physical incapacity, or none at all, can still be worthy of help.

An "atomic" approach to dis/ability assessment
Due to the difficulties occurring in disability and ability assessments, an "atomic" approach was developed: first disaggregating the entire process into its smallest possible components (atoms) and then seeking a method for reintegrating the information. Basic "atomic" units cover the structural components of diagnostic confirmation, medical impact assessment, work characterization (disaggregating the job into its components) and work demand indices. In the next step of integration any individual component may be associated with several additional items, in order to cover new situations. The proposed approach is considered applicable in individual cases and useful for the development of guidelines, and its application is recommended because there is no consensus on the proper methods to be used.

Contribution of psychotechnical tests to the evaluation of aptitude of mentally retarded adults employed in a protected work environment

This study compared psychomotor ability between mentally retarded adults (or adults with psychological problems) working in a protected environment and adults employed in an unprotected occupational environment, and assessed the use of psychotechnical tests by occupational physicians for the selection of job orientation, for making decisions on the aptitude of mentally retarded adults and for proposing job adjustments. Concrete or performing comprehension as well as reaction time (RT) to simple and complex auditive and visual stimuli were studied in 115 retarded male adults and 407 non-retarded workers. The retarded subjects showed significantly lower results than controls. Overall results are interpreted. Individually, the usefulness of psychotechnical tests as a tool among others for the evaluation of the aptitudes of a retarded worker, as well as their limits in occupational medicine, are discussed.

Disabled with work-related injuries (DWRI) and disabled with general disabilities (DGD) form the two largest groups of disabled in Israel. Although both groups are covered by National Insurance legislation, there are some marked differences between them. Thus, DWRI are granted a disabled-person status on the basis of the injury itself, while DGD have to prove that the disability affects their capacity to work. DWRI receive special grants for lodgings, equipment and a personal car; there are no such grants for DGD. There is special legislation for disability-related medical treatment of DWRI, but not for DGD. The maximum monthly disability pension for DWRI (e.g., for a 100% disabled independent) at the beginning of 1995 was ILS 10,632, vs. ILS 886 for DGD (ILS 1.00 is worth about USD 0.33). Income-tax (and other tax) benefits for DWRI are also much higher for DGD. A policy aimed at minimizing the present inequalities is proposed. Its main points are: (a) the adoption of a uniform definition of what a disabled person is; (b) professional rehabilitation for all - returning to work and assistance in adaptation to the job; (c) provision of adequate pensions for all.

Regulation implementing the Disabilities Act (No.180 of 1992). Among its provisions, it includes the following ones relating to occupational matters: it empowers the Ministry of Health to set up programmes for the prevention of biological and environmental hazards and of accidents in the workplace, and to carry out epidemiological studies to discover risk factors for disabilities; it empowers the Ministry of Labour to set up programmes for the professional rehabilitation and training and the employment of persons with disabilities; it sets up CONADIS (Consejo Nacional de Discapacidades, National Council for Disabilities), and it establishes the structure and responsibilities of this organization; it establishes the modalities of the evaluation and registration of disabilities; it introduces modifications regarding disabilities into the 1986 Regulations concerning the safety and health of workers and the improvement of the working environment (CIS 89-1065).

The purpose of the French Law dated 10 July 1987 on handicapped workers is to integrate them into the workforce or keep them at work. With a view to determine an employment policy for this population, a good knowledge of its characteristics is essential. COTOREP (Technical Committee for Vocational Orientation) is the best observation post as it is the only structure authorized to recognize the status of handicapped worker. A study of the requests received by the COTOREP of Isère (France) during the year 1990 enables us to analyze the social, medical and professional characteristics of handicapped workers in this Region. A multivariate analysis enables us to define five types of handicapped workers. This will help managers to take charge of, integrate and follow up each group, and COTOREP to locate the job most suited to the handicapped job-seeker.

Personal risk assessment under the Americans with Disabilities Act - A decision analysis approach
The "Americans with Disabilities Act" (ADA) requires employers to try to find appropriate jobs for handicapped persons. Pre-employment medical examinations must take account not only of a person's immediate ability to perform a task, but also of possible future risks associated with the work. A decision analysis model was designed and implemented to facilitate job placement decisions. It identified relevant input parameters such as typical probability and severity of adverse outcomes for three targets - the worker, the public, and productivity. In addition, specific factors that differentiate this decision from "typical" decisions are identified, including a person's health history, job site risk modifiers, and possible accommodation methods. Relevant societal values are also integrated. The system also includes a mathematical component to quantitatively integrate these input parameters for any number of events and modifiers to yield the risk of substantial harm.

Homework - A case for social protection

Wage work done at home has existed alongside factory and office work for more than a century, and although its full extent is unknown, it is widespread and possibly increasing. Performed mainly by women and the disabled, it can also be a concealed form of child labour. It is attractive to employers for its flexibility and to workers for its income opportunities, but it is poorly paid, involves long hours, offers little or no social protection and can undermine the position of other workers. This article examines all main aspects of the subject and advocates increased legal protection.

Work and life of the handicapped, together with their influence on the health
Shintai shōgai-sha no rōdō to seikatsu oyobi sono kenkō e no eikyō ni tsuite [in Japanese]
A questionnaire survey concerning their life and work was conducted among 97 persons handicapped by cerebral paralysis. In most cases, neck-shoulder-arm pain was closely associated with the handicapped persons' work because they have been forced to keep the same unnatural posture for a long time during the work. Most of he surveyed handicapped workers who suffer from progressive pain were in their 30's.

Workspace for disabled people
The investigation embraced a group of young people with motor dysfunction of lower extremities. Seventeen measurements were taken of 32 boys and 45 girls aged 15-18. There are significant differences in functional dimensions of the arm between the healthy and the disabled populations. The differences clearly indicate that there is a need for different shaping of space in which people of limited motor efficiency must function. A method based on Das and Grady's concept was used in determining workspace. This method was modified by the author. The method allows determination of workspace by means of several simple anthropometric measurements. Workspace for young people investigated was determined for the sitting position in transverse and sagittal planes. The results of the investigation constitute data for design and estimation of workplaces for disabled young people with motor dysfunction of lower extremities. The results can also be used to determine functional space in classrooms, school workshops, and educational and rehabilitation centres.

Handicap discrimination considerations in treating the impaired worker: Drugs, alcohol, pregnancy, and AIDS in the workplace
In dealing with job applicants and employees, administrators must comply with handicap and pregnancy discrimination statutes, union collective bargaining agreements, and any state or local statutes specifically directed at protecting certain classes of handicapped persons. The employer must make reasonable accommodations which allow the disabled worker to perform the job, although the employer need not implement accommodations which are an undue hardship. Similarly, if the impaired employee cannot safely perform the essential functions of the job, he or she need not be hired or retained. Employers must also recognize their responsibilities and liability under their health benefit plans and other liability statutes, and attempt to minimise their exposure.

Auxiliary technical devices for handicapped workers in manufacturing processes
Technische Arbeitshilfen für Behinderte im Fertigungsprozess [in German]
A project is described which aims at the development of modules adapted to the types of handicap that workers suffer through accidents or disease. These modules can be easily attached to machines. The auxiliary technical modules necessary in each case are identified by determining the capacities of the handicapped and the demands of the machines they used to operate. Numerically controlled machine tools and fork-lift trucks are used as examples to illustrate the method.
