Abstract This review focuses primarily on the US experience in drug and alcohol testing in the workplace. The association between drugs and alcohol on one hand and occupational injuries and other adverse employment outcomes on the other is discussed in detail. Cost-benefit issues are discussed. Caution should be exercised in transposing results from one setting to another. Relatively little is known on whether drug testing in the workforce is effective in reducing drug usage.
