Abstract Accident analyses carried out by enterprises in Brazil too often limit themselves to simply pointing fingers or hastily assigning human error to the victims or their fellow workers. This guide explains how to analyze accidents and adverse events in order to better understand the underlying causes and make honest appraisals of true responsibilities. It presents a four-step approach: data collection; analysis of the information; identification of means of control; action plan.
