Abstract The aim of this study was to examine factors influencing the medical care given to victims of incidents at sea, highlighting the difficulties of access and rescue. The 38 incidents that occurred at sea during 2002 in the Basque region of Spain requiring the intervention of rescue services were analysed. Incidents at sea have specific characteristics which can affect medical services: the scene of the incident is often dangerous and the access to the victims is difficult, requiring the intervention of rescue services which considerably delays medical assistance and transport. For these reasons, it is particularly important to provide rescue personnel with basic training in emergency medical care and in the handling of victims with traumatic injuries.
