This questionnaire has been designed to evaluate the effectiveness of SOLVE. It consists
of 70 questions on knowledge of and attitudes towards the five subject areas covered by
SOLVE. Course participants will fill in the questionnaire before taking the course. The
data given will remain entirely anonymous.
If you are a course participant, you will receive a code number before the course begins.
When you have finished the questionnaire but before you have "saved" your data, you will
enter this code number and print the last page of the questionnaire as a confirmation page.
Please either send the confirmation to the course director or bring it with you on the first
day of the course.
Please be sure to click the "save" button on the last page of the questionnaire once you
have printed the confirmation.
If you do not have access to the Internet you may be able to complete the questionnaire when
you arrive to take the course. Please contact your course director if this will be neccesary.
