Tool for completion of the General Survey Report Form (Form in WORD format)

In order to assist in completing the Report Form for the General Survey, the Office has prepared this version in WORD format which enables its completion electronically.

This form gives you access to the text of the Conventions and Recommendations.
To activate the links you have to do the following:
Open WORD and go to Tools -->Macro --> Security;
set the Security Level to Medium;
now open the document;
in the pop up window click Enable Macros;
inside the document double click the link to go to the text of the corresponding instrument.
