Approves the attached "Form of Notification about Employment Contract, the Rules of Incorporating it in the Electronic Information System, the Information Form Sent to Employer Related to the Registration of the Notification on the Employment Contract, also the Rules Related to the Real-time Receipt of Data on the Registered Notification about the Employment Contract", which introduces, inter alia, the forms of notification on employment contract (with the attached forms in appendices 1-5), the procedure of incorporation of the notification in the electronic system, the content of the notification, the notifications about employment contract in relation with changes made in the contract, with termination of the contract (with the attached forms in appendices 6-8).
