Management of blood and body fluid exposures in police staff
Police service staff are at risk of occupational exposure to blood and body fluids with the consequent risk of blood-borne virus (BBV) infections. The objective of this study was to examine the types of occupational exposure incidents experienced by Scottish police service staff and to evaluate the post-incident management provided by their occupational health services. Data were collected on the circumstances and the post-incident management of each incident reported to occupational health services over 12 months. An expert panel reviewed the post-incident management provided by the occupational health service. The panel considered that the majority of cases of occupational exposure incurred little or no risk of BBV transmission. In general, the expert panel assessed the post-incident management provided by the occupational health service units serving the police as adequate and appropriate. However, some concerns were raised in relation to a small number of incorrect risk assessments and an inconsistent approach to hepatitis C virus follow-up blood testing.

Knowledge, attitudes, and behaviour of students from a medicine faculty, dentistry faculty, and medical technology vocational training school towards HIV/AIDS
With increasing numbers of HIV/AIDS patients, physicians, dentists, and nurses taking care of these patients should have sufficient knowledge of the disease and consequently adopt suitable precautionary behaviour. The aim of this study was to examine the level of knowledge about HIV/AIDS among students from a medicine faculty, dentistry faculty and medical technology vocational training school, to investigate attitudes and behaviour, and possible differences between first-year and final-year students. A questionnaire was administered to 357 students. Findings are discussed. Overall, they highlight an obvious lack of education among all three student groups.

Oscillating migration and the epidemics of silicosis, tuberculosis, and HIV infection in South African gold miners
Hundreds of thousands of men from rural areas of South Africa and neighbouring countries have come to seek work in the gold mines. They are not immigrants in the usual sense as they work for periods in the mines, go home, and then return. This is termed oscillating or circular migration. Today, there exist serious interrelated epidemics of silicosis, tuberculosis, and HIV infection in the gold mining industry. This article discusses the role of oscillating migration in fuelling these epidemics, by examining the historical, political, social and economic contexts of these diseases.

The ILO Programme on HIV/AIDS and the World of Work (ILO/AIDS) plays a key role in the HIV/AIDS global response through workplaces. The strategic objectives of ILO/AIDS are to promote national policies and legislative frameworks on HIV/AIDS and the world of work based on the principles of the ILO Code of Practice on HIV/AIDS and the World of Work (CIS 03-444) and the international labour standard on HIV/AIDS and the world of work (proposed for adoption at the 2010 International Labour Conference). A second objective is to strengthen the HIV prevention and social protection programmes including employment creation in the world of work, focusing on workers at risk in key economic sectors in the formal as well as the informal economy. These reports on HIV/AIDS and the world of work were prepared as discussion material for the 99th Session of the ILO Conference in 2010.

Occupationally acquired infections among health care workers: Bloodborne transmission

Due to the contact with infectious patients, health care workers are at risk of work-related infectious diseases. This article reviews literature data on bloodborne transmissible infections (hepatitis B and C, HIV and other) and describes the preventive measures and the measures to be taken by health care workers in case of possible infection due to needle-stick injury or contamination due to skin, eye or oromucosal damage. At its 298th Session in March 2007, the ILO Governing Body decided to place an item on HIV/AIDS and the world of work on the agenda of the 98th Session (2009) of the International Labour Conference for a double discussion leading to the adoption of an autonomous Recommendation. On 18 June 2009, the International Labour Conference decided that an item entitled "HIV/AIDS and the world of work" was to be included in the agenda of its next ordinary session for second discussion with a view to the adoption of a Recommendation. This document presents the proposed Recommendation and the Office commentary on the proposed text.

ACOEM guidance statement - HIV and AIDS at the workplace
This guidance statement of the American College of Occupational and Environmental Medicine (ACOEM) addresses general issues of HIV and AIDS in the workplace. AIDS and HIV present tremendous challenges to infected individuals seeking to remain productive in the workplace, to employers coping with the special needs of such individuals and to physicians who treat and counsel exposed or infected personnel. Occupational physicians should strive to ensure that employers are familiar with legislation and guidelines protecting the rights of infected employees, and adopt workplace policies applying to employees with HIV infection or AIDS. Where there is potential for occupational HIV exposure, occupational physicians should assure that adequate training is provided concerning exposure, prevention and treatment.

HIV/AIDS, reproductive and sexual health, and the law
This literature survey on the HIV/AIDS pandemic argues that the law is a frequently overlooked tool for addressing the complex practical and ethical issues. The law intersects with reproductive and sexual health issues and HIV/AIDS in many ways. Well-written and rigorously applied laws could benefit persons living with (or at risk of contracting) HIV/AIDS. Access to reproductive health services should be a legal right, and discrimination based on HIV status should be prohibited. Laws against sexual violence and exploitation, which perpetuate the spread of HIV and its negative effects, should be enforced. Finally, a human rights framework should inform the drafting of laws to more effectively protect health.

Deaths due to bloodborne infections and their sequelae among health care workers
Using data from the National Occupational Mortality Surveillance (NOMS) system in the United States, a matched case-control design was employed to examine the relationship between health-care employment and death from HIV, hepatitis B (HBV), hepatitis C (HCV), liver cancer and cirrhosis from 1984 to 2004. Employment in the health-care industry was associated with increased risk of death from HIV (odds ratio OR 2.27), HBV (OR 1.98) and cirrhosis (OR 1.09) among males, and death from HCV among both males (OR 1.46) and females (OR 1.22). Nursing was the occupation with the highest risk among males for HIV and HBV, but female nurses were at decreased risk of dying from HIV (OR 0.69).

The International Labour Office (ILO) is launching a process to adopt a new international labour standard aimed at bolstering the role of the workplace in the global response to the HIV/AIDS pandemic. This report lays the groundwork for discussions on this issue at the 2009 and 2010 International Labour Conferences. It is the first of its kind to provide a comprehensive overview of action taken by national governments and international organizations on HIV/AIDS, with specific reference to the world of work, and reveals that many countries have taken significant steps to address the issue of HIV/AIDS in the work of work. This CD-ROM contains the full text of the report together with the questionnaire addressed to Member States, in Arabic, Chinese, German, English, French, Spanish and Russian.

CIS 08-964 Saving lives, protecting jobs - Second report
A total of 650 workplaces in 24 countries are now involved with ILO's Strategic HIV/AIDS Responses in Enterprises (SHARE) programme, helping to inform and protect almost a million working men and women from the risks of HIV. This second report documents the work carried out in this field by ministries of labour, employers' and workers' organizations and partner enterprises. It includes a special focus on the experiences and achievements of the pioneer country projects in Belize, Benin, Cambodia, Ghana, Guyana, India and Togo. It also captures major innovations and collaborations from on-going initiatives in other countries which will help guide the SHARE programme. For the earlier report published in 2006, see CIS 07-466.

A successful workplace program for voluntary counseling and testing and treatment of HIV/AIDS at Heineken, Rwanda
Heineken Breweries launched a workplace HIV/AIDS programme at its Rwanda subsidiary in September 2001. By January 25, 2005, 736/2,595 eligible individuals had reported for counselling and HIV testing: 380/521 employees (72.9%), 254/412 spouses (61.7%), 99/1,517 children (6.5%) and 3/145 retired (2.0%). As a result, 109 HIV+ individuals were identified: 62 employees, 34 spouses, 12 children and 1 retired. In September 2003 an anonymous HIV seroprevalence survey was performed with participation rates of 69.4% for employees, 58.2% for spouses and 79.7% for adolescents. Using the survey result, the expected number of HIV+ employees was 71, which implies a programme uptake of 87.1% (62/71) in this group. Of the identified 109 HIV+ beneficiaries, 42 were on highly active antiretroviral treatment (HAART). In November 2003 a qualitative study of awareness and health-seeking behaviour of the Heineken Rwanda beneficiaries identified key principles contributing to the success of this programme.

CIS 08-1457 Bloodborne facts - Protect yourself when handling sharps
A needlestick or a cut from a contaminated scalpel can lead to infection from hepatitis B virus or human immunodeficiency virus which causes AIDS. Although few cases of AIDS have been documented from occupational exposure, approximately 8700 health care workers each year contract hepatitis B. About 200 will die as a result. Aimed at health care personnel, this information sheet explains how to avoid the risk of cut and stick injuries (disposing of sharps in puncture-resistant containers, handling of sharps containers).

CIS 07-527 Know more about HIV/AIDS: A toolkit for workers in the informal economy
This toolkit provides easy-to-use materials on HIV and AIDS for the benefit of workers in the informal economy. It is based on the ILO Code of Practice on HIV/AIDS and the World of Work which sets out fundamental principles for policy development and practical guidelines for action in the following key areas: prevention of HIV and AIDS; management and mitigation of the impact of HIV and AIDS on the world of work; care and support of workers infected and affected by HIV and AIDS; elimination of stigma and discrimination on the basis of HIV status. The three sections of the toolkit cover: information and exercises on understanding HIV and AIDS and their consequences; rights, responsibilities, stigma and discrimination; and AIDS-related services for prevention, care and support.

CIS 06-1497 Update: The SOLVE approach to addressing psychosocial problems at work
SOLVE is an interactive educational programme designed to assist in the development of policies and actions to address psychosocial issues at the workplace. Stress, alcohol and drugs, violence (both physical and psychological), HIV/AIDS and tobacco, all lead to health-related problems for the worker and lower productivity for the enterprise or organization. Taken together, they represent a major cause of accidents, fatal injuries, diseases and absenteeism at work in both industrialized and developing countries. SOLVE focuses on prevention in translating concepts into policies and policies into action at the national and enterprise levels. This document summarizes the SOLVE concept, its methodology and its implementation at the place of work. Includes a list of SOLVE training courses for managers, workers and counsellors.

Occupational seroconversions through HIV and HCV among health care professionals in France, as of 31 December 2004

This article provides the results of a survey of human immunodeficiency virus (HIV) and hepatitis C virus (HCV) contamination in occupational settings carried out by the French Health Monitoring Institute (Institut de veille sanitaire) and the results of a study by GERES (study group on health care workers' exposure to infectious agents) on occupational HCV contamination risk factors. No new cases of occupational HIV contamination were reported among health care personnel between 1997 and 2004 (a total of 13 HIV seroconversions were reported up to 1997). Between 1991 and 2004, 54 cases of hepatitis C conversions were reported among health care personnel. Results of the studies suggest that close to half of the HIV or HCV seroconversions occurring in occupational settings could have been avoided had standard precautions been observed.

Human acquired immunodeficiency syndrome (AIDS) was first described in the early 1980s. Advances in recent years have changed both the type of care and the prognosis for this infection. The integration of persons suffering from this condition into working life is not always easy, even though considerable progress has been achieved. This review article on AIDS addresses the question of HIV infection and work, with an overview of the following main aspects: HIV infection (epidemiology, transmission, diagnosis, clinical features, treatments, prevention of infection); workplace issues (HIV/AIDS testing, vaccination, occupational exposure, high-risk groups); prevention of occupational hazards; HIV infection and employment.

Business and HIV/AIDS: Commitment and action. A global review of the business response to HIV/AIDS 2004-2005
This report provides an overview and summary of business perceptions and responses to HIV/AIDS. It draws in particular on the information collected by the 2004-2005 Executive Opinion Survey that forms part of the World Economic Forum's annual Global Competitiveness Report. The survey questioned nearly 9000 business executives in 104 countries. Questions on HIV/AIDS addressed the perceived impact of the virus (and to a lesser extent TB and malaria) on workforces and communities, the effect of HIV on company operations and revenues, and firms' response to the disease. The report includes global, regional and industry level reviews of the impact of HIV/AIDS and the business response.

HIV/AIDS is poorly understood by enterprises - Results of a global survey

The World Economic Forum, UNIADS and the Harvard School of Public Health surveyed 9000 enterprise managers in 104 countries to investigate their understanding of the impact of HIV/AIDS on their enterprise. According to the report of the survey, published under the title "Business and HIV/AIDS: Commitment and Action" (see CIS 06-461), most of the managers questioned, even those in the countries most affected by HIV/AIDS, did not know the rate of infection among their employees. It is suggested that it is because of this ignorance that so little effort is made to prevent HIV infection. Only 4% of enterprises in Eastern European have a section on AIDS in their health promotion programme, and this rate falls to 2% in the Baltic countries, Russia and the Ukraine, where the rate of infection is high. It is feared that HIV/AIDS will spread from Eastern to Western Europe, a fact not foreseen by Western European business leaders. Only a few large German enterprises, such as those in the automobile industry, have taken an interest in this question.

SOLVE is an interactive educational programme designed to assist in the development of policy and action to address psychosocial issues at the workplace. Stress, alcohol and drugs, violence (both physical and psychological), HIV/AIDS and tobacco all lead to health-related problems for the worker and lower productivity for the enterprise or organization. Taken together, they represent a major cause of accidents, fatal injuries, disease and absenteeism at work in both industrialized and developing countries. SOLVE focuses on prevention in translating concepts into policies and policies into action at the national and enterprise levels. This training package (folder and CD-ROM) provides the foundation for a five-day interactive training course with a goal to give participants the knowledge and skills to formulate a comprehensive policy and strategies to address these issues in the workplace. Patients at health care centres should be effectively protected against viral infections (hepatitis, HIV) transmitted by infected health care personnel. While the occupational physician is responsible for health and safety at work rather than for protecting the patient, they must act with regard to the well-being of the patient. In Germany, no guidelines have been issued for the protection of patients which could serve as a guide to occupational physicians, infected health care workers and employers. Nevertheless, recommendations published by scientific institutions provide sufficient guidance for rational measures to be drawn up to allow infected workers to continue in their occupation and to enable employers to carry out their duties in accordance with labour laws. Unfortunately, those in charge often make decisions on an irrational or emotional basis, leading to discrimination and injustice for health care workers. It is argued that a legal directive is urgently needed.

Occupational exposure to human immunodeficiency virus (HIV) - How can we reduce the risk?
The aim of this study was to analyse occupational exposure to potentially infectious body fluids among health care workers (HCWs) in a region of Poland. Nurses were the most commonly exposed category of HCWs. Needle-sticks were responsible for 73.6% of the incidents, while recapping a needle was the cause of exposure in 6.9% of the incidents. Prophylaxis with antiretroviral drugs was necessary in 22.8% of the cases. Among 189 registered nurses, 66 performed invasive procedures without any personal protective equipment. As many as 60.3% of exposures to potentially infectious material result from non-compliance with the relevant recommendations. Continuous education and training is critically needed to prevent occupational exposure to blood-borne infections among HCWs.

Many employees are also unfamiliar with the UN's workplace policy on HIV and AIDS, which is aimed at eliminating the HIV-related stigma and discrimination that are still all too common in workplaces. This booklet attempts to answer questions that can make a positive difference in the lives of employees and their families: how to protect oneself and loved ones from HIV; how to talk about HIV and AIDS with one's partner or children; why be tested; when living with HIV, where can one turn, inside and outside the UN, for support and treatment; what can be done to create a compassionate workplace for all, including colleagues living with, and affected by, HIV.
