Cancer incidence of workers in the Swedish petroleum industry
In a retrospective cohort study of 4,128 men and 191 women who had worked for at least one year in the petroleum industry, there were 146 cases of cancer compared with 157.6 expected in the general population. Operators at refineries had an increased risk of leukaemia (6 cases compared with 1.7 expected). Five of the six cases had started to work at the refineries in the 1950s or later. A possible cause of this increased risk is exposure to benzene. No other significantly increased risk of cancer was found. Environmental and biological assessment of exposure to benzene among petroleum workers
Occupational exposure to benzene was measured in two gasoline marketing terminals and five major refineries in Singapore. A total of 280 workers were monitored over two years. Results revealed wide variations in exposure, from 0.01 to 13.6ppm for personal time weighted average exposure over the whole workshift. The exposure of truck drivers appeared to be the highest. The average benzene exposure for process operators was relatively low. Lowest benzene exposure was found in the laboratory technicians. As cigarette smoking is known to affect metabolism of benzene, data analyses on the relationships with environmental exposure were conducted only on 190 nonsmokers. The results showed that urinary trans,trans-muconic acid, unmetabolized benzene in urine and benzene in blood were better biomarkers for low level benzene exposure as compared to urinary phenolic metabolites in urine, such as hydroquinone, phenol and catechol. Psychosocial aspects of work and health in the North Sea oil and gas industry
Part III of this report addresses the issue of shift rotation patterns among offshore workers. Repeated assessments of sleep, mood and cognitive performance over the two-week offshore work cycle clearly demonstrated the adverse effects of a mid-cycle shift change compared to a fixed-shift schedule. Part IV presents the main findings of a survey of the perceived physical and psychosocial work environment, safety, health and job satisfaction among 1462 offshore personnel on 17 platforms. Overall, the offshore sample did not show elevated levels of stress symptoms relative to comparable onshore groups, but job types, shift patterns and installation characteristics were significant predictors of safety, work and health measures. The explosion and fires at the Texaco Refinery, Milford Haven, 24 July 1994
This report describes the causes and effects of an explosion at an oil refinery in the United Kingdom in 1994. The incident was triggered by an electrical storm which caused disturbances in several parts of the plant. A combination of failures in management, equipment and control systems during the plant upset led to the release of about 20 tonnes of flammable hydrocarbons which formed a drifting cloud of vapour and ignited. Recommendations are proposed concerning safety management systems, human factors, plant design and modification, inspection systems and emergency planning.

Associations between safety climate and emotional reactions to platform movements onboard an offshore installation
A survey of 179 workers on an offshore installation showed that, in general, emotional reactions to platform movements were not associated with the safety climate. The only exception was for instrumental protection (detection and deluge systems, temporary refuges, safety devices): workers appeared to consider these measures to be useful in the prevention of accidents or during accidents caused by platform movement effects. Associations between emotional reactions and safety involvement, and between emotional reactions and satisfaction with aspects of safety and contingency measures were affected by personnel category, offshore experience and age. Results indicate an emphasis on emotional rather than cognitive coping responses to platform movements.

The main changes in the Code relate to factors to be considered when selecting a vessel to provide the recovery and rescue arrangements required by Regulation 17.

Oxygen consumption and ventilation during simulated escape from an offshore oil platform
Oxygen consumption and ventilation were measured in 26 offshore oil industry workers performing a simulated escape from an offshore structure. Measured breathing volumes suggest that the level of physical activity reflected by the current European standard for respiratory equipment for use during escape and fire may be considerably below that likely to be met in practice. Thus, smoke hoods and other forms of respiratory protective equipment may impose physical limitations on workers trying to escape from fire. Offshore installations and escape routes should be designed to minimize exercise, and workers should be trained in the use of escape breathing equipment.

Illness absence at an oil refinery and petrochemical plant
Sickness absenteeism data for workers at a U.S. oil refinery and petrochemical plant are presented for 1986-1994. 59% of employees had no illness absence during the 9-year period; 13% had three or more absences. Frequency rate and duration increased with increasing age. Increased illness absence was associated with known health risk factors (smoking, elevated blood pressure, high cholesterol, obesity). Analysis of illness absence data may be used to identify absence trends and to develop appropriate medical strategies.

Mortality study of oil refinery and petrochemical employees
Results of a mortality study of 3803 refinery and petrochemical workers at a U.S. facility are presented. Compared to national and local populations, significantly fewer deaths were observed for all causes combined, all malignant neoplasms, heart disease, nonmalignant respiratory disease, and cirrhosis of the liver among white male employees after 10 years or more latency. The brain cancer rate for this group was nonsignificantly increased; mortality from leukaemia was consistently lower than expected. Factors considered to contribute to this overall favourable mortality include the healthy worker effect, relatively low risks related to the workplace, and the beneficial effects of continuing employment.

CIS 97-1640 European oil industry guideline for risk-based assessment of contaminated sites
This report describes a uniform approach to deciding on the corrective action requirements of contaminated oil industry sites in Europe. The approach uses principles of risk-based corrective action whereby the action is appropriately tailored to site-specific conditions and hazards. The three-tired approach involves an initial assessment of the site and collection of data for the Tier 1 assessment. Collected data are compared with risk based screening levels and other criteria. If these criteria are exceeded, further assessment is carried out in Tier 2 and Tier 3 studies. Corrective action techniques are outlined.

Thickness effect in 'relatively thin' welded joints
This report describes a study to establish the influence of the thickness of welded joints on fatigue strength over the thickness range 12-26mm. Analysis of test results obtained from a literature survey confirms that the thickness effect can be extended back to at least 16mm (the base thickness recommended in published fatigue guidance). Psychosocial aspects of work and health in the North Sea oil and gas industry
Part I of this report comprises a review of the literature relating to work and health among offshore personnel. Topics covered include: patterns of illness and injury offshore; psychosomatic complaints and minor health problems; health problems associated with offshore shift rotation; mental health and stress offshore; health behaviours and lifestyle. Part II reports on a five-year follow-up study (1990-1995) which evaluated changes in mental health and job satisfaction in offshore and onshore personnel. Results showed a significant increase in perceived workload and anxiety over the follow-up period.

Extension of the subsea isolation systems reliability database
A study carried out in 1993-94 collected failure data on 50 Subsea Isolation Systems (SSIS) and analyzed the reliability of a number of system configurations. This report extends the original database by including data for an additional 63 SSIS in 1995. The results of the data base extension are discussed, effects of the extra operating experience on the reliability of SSIS configurations are considered, and results of reliability modelling of 9 SSIS configurations are presented.

UK Continental Shelf seismic hazard
This report describes a study to assess the seismic hazard on the United Kingdom Continental Shelf. A unified catalogue of earthquakes in the study area was prepared, along with a seismic source zoning map and seismic hazard contour maps. Secondary hazards caused by earthquakes, such as slope failure and loss of soil strength, were also reviewed. The hazard maps show a wide range of seismic hazard in offshore waters; areas with above average seismic hazard are identified. The report includes an abbreviated version of the earthquake catalogue and seismic activity maps.

Associations between risk perception and safety
A self-completion questionnaire survey was carried out among employees on twelve offshore oil installations in 1994. The number of respondents was 1138. Employee evaluations of the status of safety and contingency measures were affected by physical working conditions, attitudes towards safety and accident prevention work as well as management commitment and involvement in safety promotion. These factors were also related to job stress, perceived risk and risk behaviour. There was a significant positive correlation between perceived risk and risk behaviour, but risk perception was not found to predict risk behaviour. Risk behaviour affected accidents as well as near-misses. The possibility that safety cannot be improved by changing individual risk perception is discussed.

Nouveaux outils pour la sécurité [in French]
The Internal Operation Plan (IOP) of a hydrocarbon storage facility is different from that of an industrial site such as a refinery, chemical plant or other petrochemical site. However, both types of plans must undergo changes as the relevant legislation is modified and must take into consideration the experience gained from current industrial practice. New pipeline installations, whether in the countryside or in urbanized areas, require a very precise and rigorous environmental impact study to be conducted beforehand. Hydrocarbon storage facilities must obey very strict safety rules so that workers, installations and the environment may be protected in an appropriate way. In order to help managers in their task of complying with legislation and benefitting from experience, the Gésip (Groupe d'études de sécurité des industries pétrolières = Study Group for Safety in the Petroleum Industry) has published a series of up-to-date and thorough methodological guides. This article describes three such guides, devoted to the preparation of IOPs in industrial facilities, the preparation of an impact study of pipelines and the safe management of liquid hydrocarbon storage facilities. The article also provides an overview of the organization of disaster preparedness in France.

A fibre optic sensor for flexible pipeline and riser integrity monitoring
This report describes a study to investigate the feasibility of developing a fibre-optic integrity monitoring system for flexible pipelines and risers. Data on the design, construction, installation and testing of these flexibles are presented along with an introduction to fibre optics and their use as sensors, a laboratory investigation of the application of a fibre-optic sensor to a sample of flexible pipe, and a discussion of the benefits of full-scale implementation of a distributed monitoring system.

Improving inherent safety
Report of a pilot study to assess the extent to which the concept of 'inherent safety' is being applied in the development and design of United Kingdom offshore oil and gas installations. The inherently safer concept is outlined along with its potential application offshore, reviews of the literature and current regulations, and summaries of interviews with offshore operators and design contractors. Findings indicate that the term 'inherent safety' is only just beginning to be recognized in the industry; few safety professionals seem to have a clear view as to its meaning and principles. Suggestions for the promotion of the concept in the offshore industry are put forward.
