Título Promoting safety improvements via potential human error audits

Resumen The development of a Potential Human Error Audit as a means of targeting accident prevention initiatives in the British coal industry is described. The framework for the audit is based on elements selected from research and the findings of disaster enquiries: the distinction between active and latent failures; classification of human errors as either unintentional or deliberate and skill based, rule based or knowledge based; the concept of performance shaping factors. An overview of the findings of such audits is presented. Post audit improvements at one colliery resulted in an 80% improvement in the accident rate.
