Abstract This report examines the principal causes of fatalities in SCUBA (Self Contained Underwater Breathing Apparatus) diving incidents in order to quantify the risks to which divers are exposed. Accident causes include: entanglement and entrapment; rapid ascent and air embolism; loss of consciousness for unknown reasons; reckless diving; solo diving; health prior to diving. The majority of fatalities result from two or more sequential events or contributory causes (a procedural error or equipment failure). Recommendations include adequate diver training and safe diving practices.
