Abstract Description of an accident that occurred in the tanning department of an ordnance factory causing five deaths. The investigation reveals that practical safety measures had not been taken. The report lists all statutory violations which had led to the accident, such as absence of monitoring of toxic gases at the workplace, absence of an on-site emergency plan, etc. Recommendations are made for the specific measures which should have been taken and which should be applied in the future.
