Abstract A case is presented of a worker exposed to cadmium for 9 years. There was evidence of mild proximal tubular and glomerular dysfunction, and he presented considerable weight loss and other symptoms. The value of monitoring urinary total protein as recommended by the National Institute for Occupational Safety and Health (NIOSH) is discussed. Increased beta-2-microglobulin without proteinuria but with elevated blood and urine cadmium levels raises questions about which biological indicators will prove most effective in monitoring cadmium exposed workers.
