Abstract The aim of this work was to identify common causes of carbon monoxide (CO) incidents related to appliance and system design, installation and maintenance, in order to further improve customer safety, target expenditure on incident prevention and to identify further research work. A national data collection scheme was established for piped natural gas and LPG CO incidents which occur within the United Kingdom. This report provides information on the 27 incidents collected in this national data collection scheme for the period 2004/05.
