Abstract The aim of this work is to identify common causes of carbon monoxide (CO) incidents related to appliance and system design, installation and maintenance in order to further improve customer safety, target expenditure on CO incident prevention and identify further research work. Data on all piped natural gas and LPG CO incidents that occurred in the United Kingdom from 1st April 2003 to 31 March 2004 were collected and analysed. During this period, there were 36 domestic piped natural gas incidents and two LPG incidents reported. Findings are discussed.
